In TOOTH FAIRY, Dwayne Johnson plays Derek Thompson a.k.a. “The Tooth Fairy,” a hard-charging minor league hockey player whose nickname comes from his habit of separating opposing players from their bicuspids. When Derek discourages a youngster’s hopes, he’s sentenced to one week’s hard labor as a real tooth fairy, complete with the requisite wings and magic wand.

To enter the sweepstakes, complete the Game On! The Ultimate Wellness Challenge:

1. Download and print out the Game On! Tracker
2. For the next two weeks, keep score of your “Energy In” and “Energy Out” goals
3. “Energy In” goal is to make three healthy nutrition choices each day.
4. “Energy Out” goal is to get up and move for at least 60 minutes each day.
5. Keep score of your nutrition and activity choices on the tracker each day
